Death March! is a real skill and preparation check for Walked the Path, and several combat challenges (Can't Touch This!, Even Odds, Kaer Morhen Trained) punish mistakes. Most of the list is quest completion and counters rather than one brutal boss, but the hardest difficulty clear and a clean Death March ending still gate the top of the list.
